O'Donoghue's unhappy triad -MRI

Altered contour of the body and posterior horn of medial meniscus with torn fragment in the intercondylar notch and flipped over fragment anteriorly as well suggesting bucket handle tear of the medial meniscus. There is discontinuity of fibers at the femoral insertion of ACL with altered contour of the rest of the ligament suggesting ACL disruption. There is increased fluid in the knee joint with distension of the supra-patellar bursa. Increased  signal and thickening is noted in the medial collateral ligament suggesting  disruption. These three pathologies together in this patient with twisting injury constitute O'Donoghue's unhappy triad.
